Follicle Stimulating Hormone (FSH) Chemiluminescent microparticle immunoassay (CMIA) is a test for the quantitative estimation of the reproductive hormone FSH in serum and plasma. This test will determine the level of FSH in the appropriate sample of serum or plasma. Follicle-stimulating hormone (FSH) is a hormone associated with the reproductive process and the development of eggs in women and sperm in men.

FSH is a hormone secreted by the anterior pituitary gland. While it plays an important role in female reproductive function, it is found in both men and women. It helps women to release eggs and men to produce mature sperms. Insufficient quantities of this hormone in females make it difficult for the production of eggs and subsequently to get pregnant.

The test for FSH and its correlation with other hormone tests such as luteinizing hormone, testosterone, estradiol, and/or progesterone is used to:

In females:
  • Assess the cause of infertility
  • Detect diseases related to abnormal function of the ovaries
  • Help in the diagnosis of pituitary or hypothalamus disorders
  • Assist in the diagnosis of menstrual irregularities and related disorders such as polycystic ovarian disease (PCOS)
  • Ascertain menopause.
In males:
  • Evaluate the cause of a low sperm count/infertility
  • Help in detecting underdeveloped testicles
In children:
  • Helps in the diagnosis of early or delayed puberty.
  • Diseases associated with the hypothalamus, the pituitary gland, the ovaries or testicles, or other systems.
  • Assess the difference between normal patterns of development and true disease.

The test results of FSH serum levels are usually related to the results of other hormone tests, such as LH, estrogens, and/or testosterone. Although a high or low FSH is not an accurate diagnosis for an infertility workup, it can shed light on its cause.

In females,
High levels of FSH and LH are suggestive of:

  • Developmental defects such as failure to develop ovaries, chromosomal disorders like Turner syndrome, etc.
  • Damage to the ovaries due to radiation exposure, chemotherapy, etc.
  • Polycystic ovary syndrome (PCOS)
  • Adrenal disease
  • Thyroid disease
  • Ovarian tumor
  • During menopause

Low levels of FSH and LH are indicative of:

  • Absence of ovulation
  • Pregnancy
  • Ovarian cancer.

In males,
High FSH levels can be due to:

  • Viral infection (mumps)
  • Trauma
  • Radiation exposure
  • Chemotherapy
  • Autoimmune disease
  • Failure to develop normal gonads
  • In children,
    Higher levels of FSH and LH are an indication of early puberty, which can have following causes:

    • Central nervous system tumor
    • Brain injury
    • Meningitis, encephalitis, etc.

    Low levels of LH and FSH can be seen in delayed puberty that includes following causes:

    • Dysfunction of the ovaries or testicles
    • Hormone deficiency
    • Chronic infections
    • Turner syndrome
    • Klinefelter syndrome
    • Eating disorder.
